Provident American Insurance Company strongly believes in protecting the confidentiality of our customers and in the security of the information we collect about you. This noticedescribes our policy on privacy and how we treat the information we collect about you. This notice refers to Provident American Insurance by using the terms “us’, “we”, or “our.”

We collect and use information about you and other applicants for many reasons, including to help identify you, evaluate your application, service and manage your policy and premium payments, and evaluating benefit claims. We get most of the information directly from you. The information you provide us when applying for insurance or when filing a claim for benefits generally provides the information we need. If we need to verify information you provide or obtain additional information we may obtain information from third parties such as adult family members, employers, other insurers, consumer reporting agencies, physicians, hospitals and other medical personnel. Information collected may relate to your finances, employments, health, avocations or other personal characteristics as well as transactions with us or with others, including our affiliates.

Nonpublic Information we collect is treated in a confidential manner. Our employees are required to protect the confidentiality of information and may access information only when there is an appropriate reason to do so. Only those persons who need to perform their job responsibilities are authorized to have access to information. We also maintain physical, electronic and procedural safeguards to protect information that comply with applicable law. Employees are required to comply with our established policies and those who violate our policies and procedures regarding privacy are subject to disciplinary action. We regularly review our security standards and practices to protect against unauthorized access to information. We also require third parties to whom we disclose nonpublic information to limit their use of the information in a manner consistent with our privacy policy and to establish information security procedures.

We may disclose information when we believe it necessary for the conduct of our business, or where disclosure is required by law. For example, information may be disclosed to others to enable them to provide business services for us, such as helping evaluate requests for insurance benefits, for audit or research purposes, or to law enforcement and regulatory agencies to help us prevent fraud. Information may be disclosed to our Affiliate Company or companies outside of Provident American that provide services for us, such as other insurers and consumer reporting agencies. We also may make other disclosures as permitted by law. Information may also be shared with our affiliate so that they may offer you products and services. We do not make any other disclosures of information to other companies who want to sell their products or services to you. For example, we will not sell your name to a catalog company.

Generally, upon your written request, we will make information available for your review. Information collected in connection with, or in anticipation of, any claim or legal proceeding will not be made available. If you notify us that the information is incorrect, we will review it. If we agree, we will correct our records. If we do not agree, you may submit a short statement of dispute, which we will include in any future disclosure of information.

Should you decide to terminate your business with Provident American or its affiliate, we will continue to adhere to the privacy policies and practices described in this notice to the extent we retain this information. We may amend this privacy policy at any time and will inform you of changes as required by law. In addition to any other privacy notices we may provide, a recently enacted federal law requires us to provide this summary of our privacy policy once each year.
