How Can the Proceeds Be Used?
Let Us Help You Take Care Of It
Our Critical Illness pays you a lump sum benefit upon diagnosis of one of the following events:
• Cancer not in-situ
• Cancer in-situ (reduced benefit)*
• Heart Attack
• Stroke
• Major Organ Transplant
• End Stage Renal Disease
• Paralysis
• Coma
• Major Burns
• Accidental Loss of Sight
• Accidental Loss of Hearing
• Accidental Loss of Speech

*For a first ever diagnosis the benefit amount is equal to 10% of the current face amount

• Medical cost not covered by insurance
• Co-payments and deductibles
• Alternative treatments
• Loss of income -Disability Insurance only pays part of your former income amount
• Bills, mortgage, and debts don’t go away just because you are ill
• Transportation for treatment

Return of Premium


This Feature Creates a No Loss Situation
If the policy has not terminated prior to the Insured’s death, then a benefit equal to the sum of all premiums paid on this policy to date minus any benefit paid will be paid upon the Insured’s death to the Beneficiary, if alive or to the Beneficiary’s estate if not.

Reduced Benifits


Beginning the Policy year immediately following the Insured’s 65th birthday or 5 years from the policy date, whichever is later, the amount payable for the remaining years the policy is inforce will be 50% of the amount otherwise payable.

Effective Date

Insurance will become effective only after receipt of application, required premium, approval by Provident American and before any change in insurability of the applicant. Provident American reserves the right to reject any application which does not meet its underwriting requirements.
